STREET MANAGEMENT - PET AGREEMENT

This agreement is attached
to and forms a part of the Rental Agreement dated _______________ between
Lee Street Management, Owners, and ___________________________________,
Tenants.
Tenants desire to keep
the following described pet(s):

in the
dwelling located at ____________________________________ that they
occupy under the Rental Agreement referred to above. Because
this agreement specifically prohibits keeping pets without the Owners'
permission, Tenants agree to the following terms and conditions in
exchange for their permission:
1) Tenants
agree to keep their pet under control at all times.
2) Tenants
agree to keep their pet restrained and tethered, when it is outside
their apartment and on any Lee Street Management property. Lee Street
Management does not extend its restraint and tethering requirement
beyond it's own properties. See item #3 below for pubic requirements.
3) Tenants
agree to adhere to local ordinances, including leash, licensing and
vaccination requirements.
4) Tenants
agree not to leave their pet unattended for unreasonable periods.
5) Tenants
agree to provide proper care and feeding of their pets and ensure
that their pets are treated in a lawful and humane manner at all times.
6) Tenants
agree to
clean up after their pet and to dispose of their pet's
waste properly and immediately.
7) Tenants
agree not to leave food or water for their pet or any other animal
outside their dwelling where it may attract other animals.
8) Tenants
agree to keep their pet from being unnecessarily noisy or aggressive
and causing any
annoyance or discomfort to others and will immediately remedy
any complaints made either directly from a neighbor or indirectly
through Lee Street Management. Tenant agrees to notify Lee Street
Management immediately if their pet, or the pet of any other tenant,
behaves in an aggressive or threatening manner that may expose any
person to danger or harm.
9) Tenants
agree to provide their pet with an identification tag.
10) Tenants
agree to place their pet's offspring within eight weeks of birth.
11) Tenants
agree that if they cage their pets while unattended, the cages will
be kept clean, free of waste, safe, well lit, well ventilated, supplied
with water,
appropriately sized, designed for that particular pet, securely
locked and with only one animal per cage.
12) Tenants
agree to pay immediately for any damage, loss, or expense caused by
their pet, and if requested by Lee Street Management, they will add
$ ____________ to their security/cleaning deposit, which may be used
for cleaning, repairs or delinquent rent when Tenants vacate. This
added deposit, or what remains of it when pet damages have been assessed,
will be returned to Tenants within ___________ days after they have
proved that they no longer keep this pet.
13) Tenants
agree that this Agreement applies only to the specific pet(s) described
above and that
no other pet may be substituted. Tenants agree to furnish the
Owners with a picture of their pet, upon request.
14) Tenants
agree that they will never, under any circumstances, allow their pet
access to the front stairwells or lobbies of any Lee Street Management
property. Tenants will walk or otherwise transport their pets through
the rear entries and stairwells only.
15) Tenants
will either quarter their pets within, or remove their pets from,
their apartment whenever Lee Street Management must enter the apartment
for routine or emergency repairs or to show the apartment to prospective
tenants as permitted by either the CLTO
or the ELTO.
16) Tenant
indemnifies and holds Lee Street Management harmless for any damages
or claims that may arise from the tenants possession of any pet.
17) Tenants
agree that Lee Street Management reserves the right to revoke, without
recourse, permission to keep any pet should the Tenants, in the sole
judgment of Lee Street Management, violate the terms and conditions
of this agreement or if Lee Street Management decides, unilaterally,
to modify or revoke it's pet policy for the entire building.
18) Tenants
agree that they will immediately obey any request, ordinance or order
of any kind, made upon them or upon Lee Street Management, by any
governmental authority, that affects the maintenance or keeping of
their pet(s).
19) Landlord,
in his sole determination, reserves the right to perform any form
of pest or rodent extermination or control in either the common areas
or individual apartments of the building. Landlord shall have the
right to enter any premises at any reasonable time and with proper
notice to perform
such work. Tenant shall contain, remove or otherwise control their
pet(s) to accommodate any extermination effort. Tenant accepts that
such work is for the greater health, safety and welfare of the humans
living within the apartment and the building and acknowledge that
their pet('s) health, safety and welfare shall be a subrogated consideration
of the former.
